When you are a patient at Desert Spine and Sports Physicians you will be seen by one of our practitioners, Brad Sorosky MD, Susan Sorosky MD, Tima Le DO, Arthur Hatch DO, Naomi Alcock PA-C, or Allison Shannon, PA-C. We do not overbook so that we can devote the necessary amount of time to completely evaluate and treat each patient. Therefore please give us as much notice as possible if you are unable to make your scheduled appointment. This will allow us to schedule someone off of our waiting list. While we realize that unexpected events do occur, if you no-show or cancel your visit less than 24 hours in advance, you may be responsible for a cancellation fee. We look forward to seeing you at Desert Spine and Sports Physicians.

How to Prepare for Your First Appointment:

When you come to your first appointment with Desert Spine and Sports Physicians, you are encouraged to download and bring the completed New Patient form and the Demographics form to your visit. Please also review the Notice of Privacy Practices form and download and bring the completed Acknowledgement of Privacy Practices form. Finally please download and bring the completed Authorization Signature form.

Alternatively, if you are unable to download our forms or would just rather complete a paper version, please note that we do have new patient packets available at our front desk. You are welcome to arrive at least 30 minutes prior to your scheduled appointment time so that you can complete these forms in our office.

How to Prepare for Your Follow-up Appointment:

When you come to your follow-up appointment(s) with Desert Spine and Sports Physicians, you are encouraged to download and bring the completed Follow-up formto your visit. If you are unable to download our forms, please plan to arrive 15 minutes prior to your appointment in order to fill out the follow-up patient form which is available at our front desk.
